Kidney Transplantation in Patients with Focal Segmental Glomerulosclerosis

چکیده
Focal segmental glomerulosclerosis (FSGS) is a glomerulopathy primarily caused by podocyte injury that leads to focal adhesions formation between the glomerular tut and the visceral layer of Bowman capsule (1,2). h e global incidence of FSGS has been estimated at 8 cases/million/year, with 40-60% of patients developing end-stage renal disease (ESRD) within 10 to 20 years at er diagnosis (3,4). Nowadays, i ve forms of FSGS have been recognized – genetic, arising from multiple nuclear or mitochondrial gene mutations; adaptive, associated with disequilibrium in physiological load and glomerular i ltration surface; virus-associated; drug-induced; and primary (idiopathic)(5).

High cut-off haemodialysis induces remission of recurrent idiopathic focal segmental glomerulosclerosis after renal transplantation but is no alternative to plasmapheresis
A 26-year-old male experienced a recurrence of idiopathic focal segmental glomerulosclerosis (iFSGS) after his second renal transplant. Reduction of proteinuria was rapidly induced by plasmapheresis (PP) and the patient has remained in remission with a once-weekly PP regimen, which has now been continued for >3½ years. We were also able to induce remission of iFSGS in this patient by treatment ...
متن کاملSuccessful Treatment of Focal Segmental Glomerulosclerosis after Kidney Transplantation with Plasma Exchange and Abatacept in a Patient with Juvenile Rheumatoid Arthritis
Recurrent focal segmental glomerulosclerosis (FSGS) after renal transplantation is difficult to treat. Recently a series of four patients unresponsive to plasma exchange (PE) and rituximab, who were successfully treated with abatacept, has been reported. We present a 26-year-old Caucasian patient who suffered from juvenile rheumatoid arthritis and developed severe proteinuria eleven days after ...
متن کاملKidney transplantation for primary focal segmental glomerulosclerosis: outcomes and response to therapy for recurrence.
BACKGROUND For a subset of adults and children with primary focal segmental glomerulosclerosis (FSGS), proteinuria and renal dysfunction recur after kidney transplantation (KTx). Predicting recurrence and response to plasmapheresis (PP) or other interventions remains problematic. METHODS The prevalence, recurrence rate, outcomes, and treatment responses of patients with FSGS were determined a...
متن کاملRenal allograft survival in transplant recipients with focal segmental glomerulosclerosis.
Previous literature suggests that the recurrence of focal segmental glomerulosclerosis (FSGS) after renal transplantation is more common in recipients who have received an HLA-identical living-related (LRD) transplant. To address the question if FSGS patients can safely receive a 6-antigen match LRD kidney transplant, we analyzed death-censored renal allograft survival data of FSGS patients fro...
متن کاملUnsuccessful Treatment with Abatacept in Recurrent Focal Segmental Glomerulosclerosis after Kidney Transplantation
Recurrence of focal segmental glomerulosclerosis (FSGS) after renal transplantation occurs in up to 20-50% of FSGS patients and is associated with inferior allograft survival. Treatment of both primary FSGS as well as recurrent FSGS after transplantation with plasma exchange and immunosuppression is often unsuccessful and remains a major challenge as the disease still leads to end-stage renal d...
